Coronavirus Protocol

We are taking every possible precaution to protect the health of our patients and our office staff. All staff wear gloves and masks. All surfaces touched are sanitized in between patients. Only 3 patients are in the lobby at a time in order to maintain a safe 6 foot distance from one another.

The following protocol has been initiated in order to minimize the contact and spread of germs:

If you have had a fever, a cough or shortness of breath within the prior two weeks, you will be asked to stay home. You may request a telehealth visit instead. Notify us at 813-752-1336.

Co-payments are due in advance via credit or debit card. An office staff member will call you to assist in making the payment via our virtual terminal website. No cash or checks will be accepted.

 

To check in:

Call the phone number on the sign in front of the office.  Stay in your vehicle. Our medical assistant will call you to advise when to come inside.

New Patients:

Email kimberly@drtomd.com or fax 813-754-6914 your New Patient paperwork, lab and/or imaging results.

A mask is mandatory to enter the building.
No children or guests may enter the building unless you require assistance.
No food or drinks inside building.
No restroom use unless it is an emergency.
If you change or cancel your appointment without a 24 hour notice, you will be charged a $25.00 fee.
